Confidentiality and Use of Information

I have been informed that the data obtained in this interview/website will be treated as privileged and confidential and will not be released to any other party without my express consent. I do however, agree to use any of the information for research and statistical analysis, as long as it will not provide facts that could lead to the identification of my person. Any other information obtained, however, will be used by the program staff to evaluate my needs.
